Abstract: 

Understanding anatomical variations can help physicians achieve better results in clinical practice. During routine dissection, we found variations in the branches of INTERNAL ILIAC ARTERY (IIA) of a 65-year-old Iranian female cadaver. The IIA ended by dividing to an unusual smaller anterior and a greater posterior trunk which was branched to the inferior gluteal arteries. Both pelvis and the uterine ARTERY were separated from the umbilical ARTERY in the left side. In addition, the left iliolumbar ARTERY arose from the main trunk of IIA. Although this cadaver had some uncommon variations, the findings are significant from clinical perspective.

Abstract: 

Introduction: We compared the best technique for arterial anastomosis in kidney transplantation, end-to-side anastomosis to the external ILIAC ARTERY or end-to-end anastomosis to the INTERNAL ILIAC ARTERY.Materials and Methods: A total of 38 patients with end-stage renal disease who received a kidney transplant from a deceased donor were randomized into two groups in order to undergo either end-to-end anastomosis to the INTERNAL ILIAC ARTERY or end-to-side anastomosis to the external ILIAC ARTERY. Length of arterial anastomosis, cold ischemia time, hospital stay, serum creatinine level, recovery of urinary output, and surgical and clinical complications during hospitalization were evaluated. After 3 years, in the patients with a functioning allograft, creatinine clearance measure, Doppler ultrasonographic study, survival, graft loss, and erectile function were compared between the two groups.Results: Postoperative analyses showed similar recovery of urinary output (P = .39) and creatinine (P = .95) between the two groups. No differences in clinical (P = .55) and surgical (P = .80) complications or in hospital stay (P = .90) were noted. The 3-year follow-up demonstrated no differences in Doppler ultrasonography results, creatinine clearance (P = .80), patient survival (P = .22), and graft loss (P = .72). Erectile dysfunction was similar, being related only to pre-operative medical history and age. Conclusion: Both techniques showed similar results in short- and long-term follow-ups. Larger prospective studies are warranted to clarify the risk of renal ARTERY stenosis and development of erectile dysfunction.

Abstract: 

Background and aim: INTERNAL ILIAC ARTERY aneurysms are rare but potentially fatal and can be mistaken with more common medical disorders. Most ILIAC aneurysms occur in conjunction with aortic aneurysms and their etiology is atherosclerotic and degeneration. This kind of aneurysm includes less than 0.5% of all intra-abdominal aneurysms. Rupture is frequently associated with an exceedingly high mortality rate. Accordingly, these aneurysms are best managed aggressively. They are treated by resection and reconstruction, proximal LIGATION and over sewing of the distal branches, coil occlusion and endovascular techniques. Currently surgical resection and reconstruction remains the gold standard for definitive management Methods: We present a 72 year old man with an old pelvic fracture history who his chief complaint was bloody discharge from right buttock. His diagnosis in the angiography was right INTERNAL ILIAC ARTERY aneurysm and during a surgery by evacuation his hematoma, the distal and proximal of ARTERY was lighted. Conclusion: Because of their rarity and their depth in the pelvis INTERNAL ILIAC ARTERY aneurysms are difficult to diagnose and tend to present late. Their symptoms easily get conflicted with other disorders and must be considered in differential diagnosis.

Abstract: 

Ruptured INTERNAL ILIAC ARTERY aneurysms (IIAA) are extremely rare, and the repair can be challenging. This case reports described an 83-year-old patient who presented at our Emergency Department with acute abdominal pain based on a ruptured IIAA. He refused open repair and only wanted to be treated as minimally invasive as possible. Therefore endovascular embolization of the IIAA was performed. Repeat angiography showed that the procedure was successful. Treatment of a ruptured IIAA by an emergency coiling procedure under local anesthesia has been shown to be effective in this reported case.

Abstract: 

Background: Abnormal placental invasion has increased parallel with persistent rise in Caesarean delivery. Management relies on accurate diagnosis and delivery should be planned at an institution with appropriate expertise and resources for managing this condition.Case: We present a case of a placenta invasion anomaly which is the major risk factors of peripartum deaths. In this case we try to explain our approach which reduces unnecessary hysterectomy rates.Conclusion: In order to avoid postpartum hemorrhage and hysterectomy protocols, our approach which consists bilateral hypogastric arterial LIGATION, Bakri balloon tamponade and If necessary methotrexate therapy can be applied succesfully.

Abstract: 

INTERNAL ILIAC ARTERY (IIA) shows frequent variations in its branching pattern. We saw variations in the branching pattern of right IIA in a male cadaver. The IIA divided into anterior and posterior divisions. The branches of the anterior division included umbilical ARTERY, inferior vesical ARTERY, obturator ARTERY. The other branches arose from posterior trunk such as iliolumbar, lateral sacral, INTERNAL pudendal, superior and inferior gluteal arteries. Knowledge of these variations might be useful for the radiologists and surgeons.

Abstract: 

Introduction: Uterine ARTERY embolization (UAE) is a well-established treatment option for symptomatic uterine leiomyomas. In cases where the INTERNAL ILIAC ARTERY, from which the uterine ARTERY normally originates, is ligated, blood flow to the uterus can be preserved through various collateral vessels. Here, we report a case of a 44-year-old patient who underwent UAE via collateral branches of the right lumbar ARTERY due to a history of kidney transplantation. Case Presentation: A 44-year-old patient was referred for UAE for leiomyomas. She had a history of kidney transplantation with right INTERNAL ILIAC ARTERY to graft renal ARTERY anastomosis performed using the end-to-end method. The right uterine ARTERY was successfully embolized through collaterals from the right lumbar ARTERY, resulting in a positive outcome. Conclusion: We present a case in which collateral circulation was recruited during UAE in a patient with absent unilateral INTERNAL ILIAC ARTERY flow due to prior kidney transplantation.
